# PFAS Compliance Uncertainty Grows for Medical Device Makers

Many current medical devices rely on per- and polyfluoroalkyl substances (PFAS).

### **Many medical devices rely on fluoropolymers, and evolving PFAS regulations are creating new compliance and supply-chain challenges.**

As regulators increase scrutiny of per- and polyfluoroalkyl substances (PFAS), [medical device manufacturers face growing compliance uncertainty](https://www.tandfonline.com/doi/full/10.1080/17434440.2026.2702016#d1e589)
. Fluoropolymers are used in coatings, packaging, insulation, tubing, guidewires, implants, and other medical-device components because they provide durability, lubricity, chemical resistance, and biostability.

Replacing these materials is difficult because few commercially available alternatives provide the same combination of properties. In addition, changing a material can trigger extensive R&D, manufacturing adjustments, supplier qualification, and regulatory review.

In Europe, a broad PFAS restriction proposal under REACH covers more than 10,000 substances and remains under evaluation. ECHA’s scientific committees have supported an EU-wide restriction with targeted derogations, including consideration of medical-device uses where alternatives may not yet be available.

The medical and pharmaceutical industry represented 7% of PFAS consumption in the European Union in 2020. ## Operational Transitions

Meeting new regulatory requirements may require manufacturers to redesign products, qualify alternative materials, modify production processes, and retrain employees. Procurement strategies may also need to change as companies seek greater transparency about PFAS content throughout their supply chains.

These transitions can be costly and time-consuming, particularly for products that require extensive validation before manufacturers can introduce material changes.

## Impact on Device Availability

Limited availability of suitable alternatives has raised concerns about whether broad PFAS restrictions could affect medical-device supply.

The EU restriction proposal includes transition periods and potential derogations for certain uses where alternatives are unavailable or require significant time for development and certification. Medical devices are among the sectors being evaluated as part of the ongoing restriction process.

Device availability also affects patient care because fluoropolymers often perform functions that are difficult to replace. Polytetrafluoroethylene (PTFE), for example, provides low-friction surfaces for guidewires and other devices that must move through blood vessels.

Fluoropolymers are also used in cardiovascular implants, catheters, tubing, wound dressings, and electrical insulation in devices such as pacemakers. FDA notes that these materials provide properties such as lubricity, electrical resistance, durability, and biostability that remain difficult to reproduce with alternative materials.

Common cardiac implantable electronic devices use fluoropolymers to navigate the vascular system. ## Finding Clarity for Compliance

Manufacturers now need to track regulatory developments closely while also understanding where and why PFAS appear in their products. Supplier coordination will be increasingly important because companies may need more detailed information about fluoropolymers, processing aids, coatings, and other fluorinated substances throughout the supply chain.

Regional differences add another layer of complexity. While Europe is evaluating a broad restriction under REACH, FDA currently takes a different approach to fluoropolymers used in medical devices.

FDA says fluoropolymers used in medical devices differ from the smaller PFAS linked to environmental and health concerns. Based on available evidence, the agency says it is not aware of harmful public-health effects from these fluoropolymers and currently sees no reason to restrict their continued use in medical devices.

This divergence illustrates one of the central challenges facing the medical-device industry. Manufacturers must respond to growing pressure to reduce PFAS use while also ensuring that replacement materials do not compromise device performance, availability, or patient safety.

Collaboration among regulators, manufacturers, material suppliers, researchers, and healthcare professionals will therefore be essential as PFAS policies continue to evolve.
